Faculty may use the “New Faculty Profile Form” to submit information for a new profile or edits for an existing one. Please choose the profile option on the form below. Once we receive your form submission, members of the OMC web team will create/update your online profile.
NEW! For faculty updating existing profiles, there is now an option to request the deletion of information within your profile.
PHOTO: This form asks for a profile photo. We like to showcase individual personality feel free to upload a professional image that showcases your unique character.
